*Los protocolos españoles solo contemplan la mitad de la biología del tumor: están pensados para el cáncer de mama HR+ estándar, y el de Miriam no lo es.*
En la práctica: el tratamiento hormonal aborda la parte del tumor, pero deja sin tratar su componente . Confirmarlo y atacarlo exige rebiopsia molecular y revisión por expertos internacionales — pruebas que la sanidad pública no cubre para un caso tan infrecuente.

Hacia dónde va
h2. El objetivo es un ensayo N-of-1 diseñado para la biología exacta de este tumor.
Un ensayo es un ensayo clínico hecho para una sola paciente: el tratamiento se diseña sobre el perfil real de su tumor, no sobre un protocolo medio. Eso permite combinar fármacos para atacar a la vez sus dos caras — la luminal (de mama) y la posible neuroendocrina.
**Las ventajas de llegar a ese ensayo:**- El tratamiento se ajusta al perfil real del tumor, no a un protocolo diseñado para la mayoría. - Se pueden aplicar fármacos adecuados a cada parte del tumor de forma simultánea. - Cada resultado que se obtiene aporta evidencia para otras personas con perfiles similares. - Miriam deja de depender de un único protocolo que no da respuesta a su caso.

Far beyond the diagnosis
*This is the most personal part of the site, and it's being written by someone who knows Miriam closely: who she is beyond cancer, what she did, and why she does what she does. It'll be ready very soon. Meanwhile, you can follow the day-to-day on her social channels or get to know the case in The science.*
Three years ago, Miriam was working full-time as a software developer and creating content about technology. Within a matter of months, her life took a radical turn.
It all began with back pain, physiotherapy sessions and GPs who couldn't find a solution. As the pain persisted and she wasn't getting answers, she decided to arrange an MRI scan herself. One Thursday in January 2024, she found the test results in her inbox: “probable vertebral metastases”. A couple of weeks later, the definitive diagnosis arrived: “Stage IV breast cancer with neuroendocrine differentiation”.

Hace tres años, Miriam se dedicaba a tiempo completo a programar y crear contenido sobre tecnología. En cuestión de meses, su vida dio un giro radical.
Todo empezó con un dolor de espalda, sesiones de fisioterapia y profesionales de la salud que no buscaban una solución. Como el dolor no remitía y no encontraba respuestas, decidió solicitar por su cuenta una resonancia magnética. Un jueves de enero de 2024, encontró en su bandeja de entrada el resultado de la prueba: «probable metástasis vertebrales». Un par de semanas después, llegó el diagnóstico final: «Carcinoma de mama con diferenciación neuroendocrina en estadio IV».

Yes. Analysis of the tumor has found specific "weak points" that could be treated with targeted therapies. These are promising hypotheses, not a guaranteed cure: that's why they must be confirmed before acting.

h2. Why doesn't this tumor fit the protocols?
On paper this is a breast cancer (HR+/HER2−). Under the microscope, ~80% of the tumor shows neuroendocrine differentiation, with and amplification. Spanish protocols recommend treating it as standard luminal and ignoring the component; yet the WHO has recognized breast neuroendocrine neoplasms as a distinct entity since its 2019 classification, and clinical literature shows this hybrid subtype () has worse prognosis and usually needs different strategies.

Sources and methodological notes
The bone-lesion biopsy (right iliac, April 2026) contained no evaluable tumour —only mineralised bone trabeculae and muscle tissue—, so the molecular profile is read from ctDNA. It is also why the advanced rebiopsy targets the soft-tissue component, where NGS performs far better than in pure bone.

⁺⁺ In neuroendocrine oncology, Ki67 is the primary grading marker: G1 (<3%), G2 (3–20%), G3 (>20%). By that logic, a high Ki67 would be compatible with a high-grade neuroendocrine component (NEC G3): a hypothesis, not the assigned grade. The confirmed histological grade is Nottingham Grade II; reconciling it is pending tumour-board review.

PET with the 68Ga-DOTATOC radiotracer, which binds mainly the somatostatin receptor subtype 2 (SSTR2). Visualizes in vivo which lesions express SSTR2 and could therefore respond to radioligand therapy (PRRT).

**Clinical meaning:** The tumor expresses somatostatin receptors — the target of radioligand therapy (PRRT). It confirms on imaging both tumor drivers (luminal FGFR1/CCND1 + neuroendocrine RB1 + SSTR) and opens a route that breast-cancer protocols do not contemplate as standard.

h2. Bone rebiopsy protocol
A single biopsy from which to obtain the most complete tumour characterisation that science currently allows —genomic, immunogenomic and epigenetic—, so it doesn’t have to be repeated. This is what the campaign funds.

14G coaxial needle into peri-lesional soft tissue, without decalcification (acid degrades DNA ~10×; if bone must be decalcified, EDTA only, never acid). Touch-prep/ROSE to confirm ≥30% tumour cellularity before allocating anything. The germline reference (blood/saliva) goes separately, any day. WES+WGS+RNA-seq are not committed to a single core: 1–2 frozen cores are reserved for genomics and transcriptomics. Serial liquid biopsies close the loop with real-time decisions.

Each treatment line has held the disease for a while before the tumour escapes again: the pattern to expect when only the hormonal axis is targeted and the neuroendocrine component is left out.

Diagnosed in January 2024 after presenting to the ER with low-back pain: vertebral metastases were found. Premenopausal. Palliative systemic treatment started in February 2024 with the goal of prolonging survival. Received radiotherapy to the spine with a good response: progressive improvement, spinal stability and no fractures.

Un cáncer de mama que se ha extendido a los huesos. Lo poco común es que se comporta como dos enfermedades a la vez: en parte como un cáncer de mama «de los hormonales» y en gran parte como otro tipo distinto (neuroendocrino). Son como dos caras del mismo tumor.

Los protocolos lo tratan como un cáncer de mama corriente y atacan solo una de sus dos caras. La otra queda sin tratar, y el tumor encuentra la manera de seguir avanzando.

Sí. El análisis del tumor ha encontrado «puntos débiles» concretos que podrían tratarse con terapias dirigidas. Son hipótesis prometedoras, no una cura garantizada: por eso hay que confirmarlas antes de actuar.

Una nueva biopsia con un análisis molecular avanzado para confirmar esos puntos débiles y diseñar un tratamiento hecho a su medida. Es lo que financia la campaña.

h2. ¿Por qué este tumor no encaja en los protocolos?
Sobre el papel es un cáncer de mama (HR+/HER2−). En el microscopio, ~80% del tumor muestra diferenciación neuroendocrina, con amplificación y . Los protocolos españoles recomiendan tratarlo como luminal estándar e ignorar el componente ; sin embargo, la OMS reconoce las neoplasias neuroendocrinas de mama como entidad propia desde su clasificación de 2019, y la literatura clínica indica que este subtipo híbrido () tiene peor pronóstico y suele necesitar estrategias distintas.

La biopsia de la lesión ósea (ilíaca derecha, abril 2026) no contenía tumor evaluable —solo trabéculas óseas mineralizadas y tejido muscular—, así que el perfil molecular se obtiene del ctDNA. Es también la razón de dirigir la rebiopsia avanzada al componente de tejido blando, donde la NGS rinde mucho mejor que en hueso puro.

h2. Hacia dónde apuntar
De todo lo anterior se desprenden varias dianas. Estos son los ejes terapéuticos candidatos que sugiere el perfil molecular, cada uno con su propia lógica de tratamiento.

Una sola biopsia de la que sacar la caracterización del tumor más completa que la ciencia permite hoy —genómica, inmunogenómica y epigenética—, para no tener que repetirla. Es lo que financia la campaña.
**Próximo paso (junio 2026): una rebiopsia ósea extendida, guiada por PET. La primera (abril 2026) no obtuvo tumor viable; esta busca asegurarlo para completar el perfil molecular y ver cómo evoluciona.**

Especificaciones técnicas de la biopsia
Aguja coaxial 14G sobre partes blandas perilesionales, sin descalcificar (el ácido degrada el ADN ~10×; si hay que descalcificar hueso, solo EDTA, nunca ácido). Touch-prep/ROSE para confirmar ≥30 % de celularidad tumoral antes de repartir nada. La referencia germinal (sangre/saliva) va aparte, cualquier día. No se comprometen WES+WGS+RNA-seq a un único core: se reservan 1–2 cores congelados para genómica y transcriptómica. Las biopsias líquidas seriadas cierran el bucle con las decisiones en tiempo real.
✱Material de apoyo a la consulta: describe lo que una sola biopsia podría extraer; no es una indicación de tratamiento. Las decisiones —dónde pinchar, cuántos cores son seguros, qué ensayos son viables— las toma el equipo médico tratante.

Cada línea de tratamiento ha controlado la enfermedad durante un tiempo antes de que el tumor escape: el patrón esperable cuando se ataca solo el eje hormonal y se deja fuera el componente neuroendocrino.

Diagnóstico en enero de 2024 tras acudir a urgencias por dolor lumbar: se hallan metástasis vertebrales. Premenopáusica. Se inicia tratamiento sistémico paliativo en febrero de 2024 con objetivo de prolongar la supervivencia. Recibe radioterapia sobre columna con buena respuesta: mejoría progresiva, estabilidad vertebral y sin fracturas.

El abemaciclib (siempre a 100 mg) se suspende el 30/03/2026 por la progresión y una hepatotoxicidad leve que se normalizó al retirarlo. Zoladex y ácido zoledrónico continúan. Sin crisis visceral.

Desde julio de 2026 está en tratamiento dentro de un ensayo en fase 1 con BG-75202, en el Vall d'Hebron. Las pruebas de imagen del cribado (julio 2026) mostraron lesiones hepáticas: la enfermedad, hasta entonces exclusivamente ósea, dejó de estar confinada al hueso. La primera rebiopsia ósea (abril 2026) no obtuvo tumor viable, así que se plantea una rebiopsia ósea extendida guiada por PET — la prueba central que financia la campaña. Estado funcional: ECOG 1.

Lo poco común es que se comporta como dos enfermedades a la vez: en parte como un cáncer de mama «de los hormonales» y en gran parte como otro tipo distinto (neuroendocrino). Son como dos caras del mismo tumor. ### **¿Por qué no basta el tratamiento estándar?** Los protocolos lo tratan como un cáncer de mama corriente y atacan solo una de sus dos caras. La otra queda sin tratar, y el tumor encuentra la manera de seguir avanzando. ### **¿Hay por dónde atacarlo?** Sí. El análisis del tumor ha encontrado «puntos débiles» concretos que podrían tratarse con terapias dirigidas. La biopsia de la lesión ósea (ilíaca derecha, abril 2026) no contenía tumor evaluable —solo trabéculas óseas mineralizadas y tejido muscular—, así que el perfil molecular se obtiene del ctDNA. Es también la razón de dirigir la rebiopsia avanzada al componente de tejido blando, donde la NGS rinde mucho mejor que en hueso puro. What's uncommon is that it behaves like two diseases at once: partly like a hormone-driven breast cancer, and largely like a different type (neuroendocrine). They're like two sides of the same tumor. ### **Why isn't standard treatment enough?** Protocols treat it as an ordinary breast cancer and target only one of its two sides. The other is left untreated, and the tumor finds a way to keep advancing. ### **Is there a way to target it?** Yes. Analysis of the tumor has found specific "weak points" that could be treated with targeted therapies. These are promising hypotheses, not a guaranteed cure: that's why they must be confirmed before acting. ### **What's needed now?** A new biopsy with an advanced molecular analysis to confirm those weak points and design a treatment tailored to her. The anomaly ## Why doesn't this tumor fit the protocols? On paper this is a breast cancer (HR+/HER2−). Under the microscope, ~80% of the tumor shows neuroendocrine differentiation, with and amplification. The bone-lesion biopsy (right iliac, April 2026) contained no evaluable tumour —only mineralised bone trabeculae and muscle tissue—, so the molecular profile is read from ctDNA. It is also why the advanced rebiopsy targets the soft-tissue component, where NGS performs far better than in pure bone.
